End-of-Life Pathway

Support to help you remain at home as you near the end of your life.

Many older people wish to remain in their own homes with their family as they near the end of their life. The new End-of-Life Pathway provides additional support to help older people remain at home if they are assessed as having 3 months or less to live.

The End-of-Life Pathway is part of the Support at Home program.
It provides up to $25,000 in short-term funding for care and support at home over a period of 12 to 16 weeks.

This care is designed to work alongside medical or palliative services provided by NSW Health. While your health team continues to manage symptom control, providers like allcare can help with day-to-day needs and emotional support.

What is the End-of-Life Pathway?

Who is eligible?

To access the pathway, a person must:

  • Be diagnosed with an illness expected to cause death within 3 months

  • Have a Karnofsky score of 40 or below (as assessed by a doctor or nurse practitioner)

  • Have their health professional complete the End-of-Life Pathway Form

People already receiving Support at Home services can request a high-priority review. If not yet on a program, a referral for urgent assessment can be submitted through My Aged Care or with support from a GP. 

Clients can use the funding to access any services available under the Support at Home program, such as:

What can funding be used for?

✓ Nursing care

✓ Help with bathing, dressing and mobility

✓ Meal preparation and light tiding 

✓ Respire for carers 

✓ Physiotherapy for pain relief 

✓ Equipment 

✓ Counselling services 

✓ Assistive technology (for example, pressure care beds)
