Our Services
Here’s what we can do for you
You’re at the centre of your care
We deliver services that are approved under the Support at Home Program and tailored to your assessed needs. Your care needs will be documented in a goal-orientated Support Plan, developed through an aged care assessment. Services must be included in your plan and align with the Support at Home service list.
Through allcare, you may access a range of services grouped into three categories under the Support at Home Program:
Clinical Supports - e.g. nursing, physiotherapy, occupational therapy.
Independence - e.g. personal care, social support, transport, respite.
Everyday Living - e.g. cleaning, gardening, shopping, meal preparation.
All services must be approved in your Notice of Decision and Support Plan.
You are supported to make informed choices about your care. Your Care Partner will help you understand which services are eligible under the Support at Home Program and how your funding can be used in line with your assessed needs.
The Added Extras
Because of our affordable and transparent pricing, allcare is able to offer a range of extra services to our clients, which many other in-home aged care providers do not. These services and programs can be included in your Support Plan and are designed to improve your quality of life and independence.
The following are some ideas for ways we can help you; however we encourage you to call us on 1300 377 074 to discuss your individual needs and services you are interested in:
-

Flexible Transport
Many in-home aged care providers will ask for up to 14 days’ notice for a transportation booking, however here at allcare we provide our clients with flexible transport on demand.
Whether you need to attend a medical appointment, you’d like to go shopping, enjoy a social outing or visit family, our flexible transport services are ready to go!
-

Specialist Dementia Care

Research has shown that you can improve the quality of life of people living with dementia by supporting them to remain living safely and independently in their own home. We are committed to helping clients and their carers, and be their full circle of support.
-

Clinical Care
Our experienced nursing, physiotherapy and occupational therapists work alongside other healthcare partners to provide services that are tailored to your individual needs. allcare can assist you with monitoring blood pressure, diabetes management including administering insulin, wound management and pressure stocking application.
-

Palliative Care
allcare is able to deliver palliative care to clients that need it. Palliative care is person and family-centred care that is provided to clients with an active, progressive, advanced disease, who have little or no prospect of cure and who are expected to pass away. Palliative care is care that helps people live their life as fully and as comfortably as possible when living with a life-limiting or terminal illness.
-

Out and About
Out & About is a program that keeps you connected with your community, doing activities you want to do safely. Examples of past activities include attending community concerts, theatre performances, or enjoying a meal in a local park. All activities are planned in line with your Support Plan and must be directly related to your assessed care needs.
-

Assistive Technology and Equipment
We support you to use assistive technology and home modifications that help you live safely and independently at home. Under the Support at Home Program, these supports are funded separately through the AT-HM scheme and must be included in your Support Plan. Examples include walking frames, shower chairs, grab rails, and ramps. All items must be listed on the AT-HM list and be directly related to your assessed needs.
“I rely on external services such as gardening and domestic assistance. My wife and I are very happy with the service we get from allcare and love my Care Partner. She is very accessible and always gets back to us.”
