We assist with household tasks, such as cleaning, laundry, and gardening, so you can relax in a comfortable and well-maintained home.

Out and About is a program that helps you stay socially connected and active. Activities may include supported outings such as attending community events or visiting local attractions. Your Care Partner will work with you to ensure these activities align with your care goals and are eligible under the Support at Home Program

Our compassionate care workers provide help with daily personal care like bathing, dressing, and medication reminders, giving you the support to live independently.

We support you to enjoy nutritious, balanced meals that meet your dietary needs and preferences. Under the Support at Home Program, meal preparation and delivery are funded services when they are part of your assessed needs and included in your Support Plan. Our Care Partners can assist with preparing meals in your home and supporting you with shopping for ingredients, where this is part of your approved services.

Our Services & Added Extras

We support you to live safely and independently at home. This may include installing safety equipment such as grab rails or personal alert systems, where these are included in your Support Plan and approved under the Support at Home Program. We can also assist with basic home maintenance tasks - such as gardening or minor repairs - that help keep your home in a liveable condition. General renovations or cosmetic upgrades are not covered.

Assistive technology includes equipment and products that help you do things more easily or safely at home. This may include items like walking frames, shower chairs, or personal alarms. Our Care Partners are trained to support you in using approved assistive technologies that align with your assessed needs and are included in your Support Plan. All assistive technology must be listed on the Support at Home AT-HM list and be directly related to your care goals.

Our qualified nurses and allied health professionals, including physiotherapists and occupational therapists, provide services that support your health, mobility, and independence. These services are delivered in line with your assessed needs and must be included in your Support Plan under the Support at Home Program. We can assist with nursing care (e.g. wound care, medication support, diabetes management) and allied health services (e.g. physiotherapy, occupational therapy) to help you maintain or regain function and reduce the risk of falls.

We offer flexible transport options to help you attend medical appointments, go shopping, or stay socially connected. Transport services may include direct transport (a car and driver) or indirect transport (vouchers for taxis or rideshare services). Your Care Partner will work with you to include transport in your Support Plan, ensuring it meets your assessed needs and complies with the Support at Home Program. Transport cannot be used for holiday-related travel, and all transport services must be documented and approved in your plan.
